Each item is individually carved after your order is placed.  This allows you to design exactly the plaque you want.  For example, the top line can state the unit and the bottom line can specify the rank and name of the individuals.  To include service dates, use the Special Instructions box.  If you do not find a template that you like, please select "Blank Plaque" from the product list and tell us exactly what you want.  Be sure to include a phone number and e-mail address in case we need to contact you.

Your name and dates of service can easily be added to the plaque.
